Membership Applications

Two types of membership are available:

• Membership runs from January 1 to December 31
• Individual Membership Dues are $75 per year
• Corporate Membership Dues are:
  • $250 per year for 1-10 professionals
  • $300 per year for 11-20 professionals; and
  • $350 per year for over 20 professionals.

You can make application and pay your dues in one of two ways:

Register and Pay Online

You can click below on "Register Now" for Individual or Corporate Membership and complete the online questions for registration. Payments will be processed on PayPal's secure server. A PayPal account is not required to pay with any credit card.

If you are applying for a Corporate Membership, the primary member should complete the online form. It is necessary for all other employees of the Corporate member to complete a separate form in order for that employee to enjoy the benefits of membership. Click here to download the form to be completed by each employee and fax the completed forms to the number shown on the form.

Register By Mail

You can click below on "Download PDF" for Individual or Corporate Membership and print out the necessary form. You can make payment in any of the ways indicated on the form. For corporate members, please complete a separate form or provide a spreadsheet with all requested information for any additional individuals to be included on the application.
